Surest is an employer-sponsored UnitedHealthcare health plan that uses copays instead of deductibles and coinsurance. Coverage for adolescent treatment still depends on your employer’s plan, medical necessity, network status, and authorization requirements.

What Is Surest Insurance?
Surest began as Bind and is now a UnitedHealthcare company. Members access care through UnitedHealthcare and Optum Behavioral Health networks.
Surest plans generally have no deductible or coinsurance. Members instead see copays for covered care through the Surest app or member website. These features can make costs easier to understand. They do not mean that every service or provider is covered.
Surest vs. UnitedHealthcare (Why the Difference Matters)
Surest uses UnitedHealthcare networks, but it is not identical to a traditional UnitedHealthcare plan. It has its own member ID card, benefit design, and claims instructions.
The Surest electronic payer ID is 25463.
Parents should provide the exact information on the Surest card. Hillcrest uses it to check behavioral health benefits, network status, authorization, and expected costs. We do not assume network participation based only on the UnitedHealthcare name.

Surest plans include mental and behavioral health benefits. Federal parity protections generally require applicable plans to manage these benefits comparably to medical and surgical care. Parity does not guarantee payment for a specific facility, level of care, or treatment duration.
Residential coverage usually requires evidence that care is medically necessary. A reviewer may consider symptom severity, safety, functional impairment, treatment history, and whether a less intensive setting can meet the teen’s needs.
Hillcrest treats concerns such as depression, anxiety, trauma, OCD, bipolar disorder, and eating-related symptoms. A diagnosis alone does not establish coverage. Records must support the requested care.
Will Surest Cover Substance Use Treatment?
Surest identifies substance use disorder care as a covered benefit category. Exact benefits depend on the employer’s plan, network, and clinical criteria. Detox, residential care, and outpatient treatment may have different requirements.
Hillcrest provides adolescent substance use treatment when our program can safely meet the teen’s needs. Our dual diagnosis treatment addresses co-occurring substance use and mental health concerns together. If medical detox is necessary, the admissions team can help families understand the appropriate first step.

Prior authorization is the plan’s review of a proposed service before treatment begins. Surest requirements generally correspond with the UnitedHealthcare network listed for that member. Residential treatment may require approval before admission.
Some members have Surest Flex. This plan design requires advance activation for a limited number of planned services. When activation applies, the member must complete it at least three business days beforehand. Otherwise, the service may not be covered.

What Parents Should Expect at Hillcrest
Hillcrest uses a phased model that moves from assessment and stabilization to deeper therapeutic work, family healing, and reintegration. The full model is designed around approximately 90 days. Each teen’s needs and authorized benefits are different.
Treatment may include individual, group, and family therapy, psychiatric services, experiential activities, and academic support. Parents receive guidance and participate in family work.
